3.  Check spelling and print out the form.   
4.  For best results new garments should be washed first to remove sizing.  
5.  Place order form inside package with garments.

SUGGESTIONS 
It is recommended that your postage include insurance sufficient to cover the merchandise within your package, as Proprintwear and/or Stencil-Pro shall not be liable for item lost in transit to our facility.  During return shipping we shall fully responsible insure your merchandise via the United States Postal Priority Mail Service, which usually requires 2-3 days.
